The Guardian Consent Form is what you have to fill out in order for someone (Grandmother, etc.) to be able to have temporary legal guardianship over the child in order to bring he/she to doctor, hospital and anywhere else that needs parents/guardians signature. The Photo Consent Form is what you would use if you were planning on using a child's photo on the internet or anywhere else in the public's view. This would have to be filled out with parents/guardians signature. The Church Camp Consent Form is an important form for your church if the church has a youth camp or any kind of camp activities.
