The Permission Slip for Field Trips/Activities form is a must before you take any trip with a group of children. The parents must read, fill out and sign the form before their child can participate in any field trip or activity. In some cases, depending upon your church, they might even have you notarize the form before returning it to the church office. The Permission for Youth to Volunteer in the Childrens Ministry form is what the parent has to read, fill out and sign before their child can volunteer to help as a Youth Worker Volunteer in any activity.
