Free Prayer Request Form and Comments - Concerns Form to download...
The form for requesting prayer below is important form to have available for the children in your Children's Ministry. Sometimes, it is hard for us to remember, but they have needs like the rest of us.
What I have noticed many times is that when they fill out their request and we pray with them it gives them faith to believe in whatever they are needing at that time. It also lets them know that someone else truly does care.
The Comments and Concerns Form is a wonderful way for children to let the Children's Ministry leaders know of any comments or concerns that they might have at that present time. The children just gets the form and fills it out with their name and phone number along with their comments and concerns and returns it to the Children's Ministry Leader. This is just another way of communicating with our children, which we definitely need to always do and to never neglect.
The Request for Prayer Form includes:
- Your Name if You Wish to Be Contacted
- Your Phone Number (Please Note if it is Unlisted)
- Current Date
- Request for Prayer or Praise Report Comments
- Check Boxes to choose Request for Prayer or Praise Report
- Check Boxes to choose Share with Pastors, May Be Shared With All Members or Please Do Not Share With Anyone Else
When these little children need something from God they have no doubt they believe God is going to do it. They also know when they submit their request for prayer that God will answer this request. This is why I know, as the Holy Bible states, we have to become as a little child to enter into the Kingdom of God.
