Free Youth Camp Registration, Counselor Registration, Waiver Forms & More...
The Youth Camp forms are a very great necessity if your church has it's own camp for the youth. The youth in the churches today need encouragement and spiritual guidance and also to have fun at the same time. Our church has a great vision and burden for our youth and we have an awesome youth camp every year! This is a very important Ministry of the church. There are lots of forms that are required for camp. There are three forms below and several pages leading to more YC (Youth Camp) forms. The YC Registration form is for the parents to fill out prior to camp for their children. The YC Counselor Registration form is for the counselor to fill out prior to camp. Every camp needs a counselor available. The YC Waiver and Medical Release form is for parents to fill out prior to camp to give their approval for their kids to attend camp and if in an emergency receive medical attention from a physician without them present if the case may be.
