The Youth Camp Volunteer Background Check form or template below is a great tool and may I point out a very important tool in your Youth Camp paperwork process that does not need to be excluded if at all possible.
One of the biggest mistakes you can make in your hiring process is skipping the background and reference checks on your potential church employee especially if they are going to be working with children.
A reference check is an opportunity to protect your youth camp, church and/or organization and also to help get to know to your potential church employee.
Information from your church background check reports should be made available to all the decision makers before the final hiring process. If the report comes back cleared and positive then great, but if you have any questions through the reference and background checks then you need to continue conversations with your potential new hire in an effort to get clarity on these questions.
There is a misconception of doing background checks on church employees in that they are done to "dig up dirt" on a potential employee, but this is not the case. The background check process is intended to get you to know your potential church employee better and gain clarity on whether they could be a good fit for your church team or not.

- Potential Employee's Name
- Potential Employee's Other Names Used
- Potential Employee's Social Security Number, Date of Birth, Driver's License, State, and Present Address
- I understand that in connection with
my application for employment, [Church Name] may use an outside agency to research
and verify the information I have provided on my application for employment.
This agency will provide a report to [Church Name].
- I understand that the outside agency will obtain
information it deems appropriate from various sources including, but not
limited to, the following: credit reporting agencies, current and past
employers, criminal conviction records, Department of Motor Vehicle records,
military records, school records, and professional and personal references. I
authorize, without reservation, any individual, corporation or other private or
public entity to furnish [Church Name] all information about me.
authorization and consent, in original, faxed, photocopied or electronic form,
shall be valid for this and any future reports and updates that may be
requested by [Church Name].

- ALL APPLICANTS UNDER THE AGE OF 18 YEARS OLD MUST PROVIDE A LOCAL
CRIMINAL RECORD. THIS CAN BE DONE THROUGH THE COUNTY SHERIFF’S OFFICE IN WHOSE
JURISDICTION THE PESON BEING SCREENED LIVES. CONTACT THE PERSONNEL OFFICE AT [PHONE NUMBER] FOR ADDITIONAL INFORMATION.
