This form is to be used to document all injuries, accidents or mishaps that take place at any facility or property of churches, etc.
If you have worked in a church then you know that there are things that happen beyond your control that require a record for liability purposes. This form will serve as an adequate tool for your church to capture the nature of the events at the time they happen and having the adequate information to back these incidents on your property.

states to report to. This should be specific in your Church Incident Policy and Procedures and clearly stated each step to take including who to give the incident report to once it is filled out and signed. Always keep a copy of the incident report in the
church files for future reference.
Church Activities Safety
Church activities should always include proper supervision and appropriate training. Church leaders can minimize the risk of accidents and injuries by planning effectively and following appropriate safety precautions.
Church activities should involve minimal risk of injury or illness to the members or participants, and should also involve minimal risk or damage to the facilities or church property. During these activities, it is the leaders responsibility to make every effort that they need to make to ensure the participant's safety. Leaders should be prepared for any type of emergency that might occur during these activities. They should be educated and know in advance just how to quickly contact local law enforcements and emergency services.
If an accident or injury occurs on Church property, leaders should:
- Apply First Aid - If a person needs medical attention or care beyond simple first aid, contact emergency medical services, the Pastor, and the parent, guardian, or other next of kin.
- In Case of a Missing Person or Fatality - Immediately notify local law enforcement authorities and cooperate fully with them.
- Provide Emotional Support
- Do Not Encourage or Discourage Legal Action Against the Church, and do not make commitments on behalf of the church.
- Gather and Collect any Witness(s) Names, Information, Accounts and Events of What Happened and Possible Photographs.
