Direct Deposit Revocation Forms
The Direct Deposit Revocation of Prior Authorization form (2) are available free below for your church or organization's Human Resources Dept...
Does your church or organization have direct deposit as a choice on their payroll and have a need to cancel these deposits for various reasons?
If yes, then you might need one of the forms below such as the Revocation of Prior Authorization for an Automatic Direct Deposit and Revocation of Prior Authorization and Authorization for Preauthorized Payments. All you have to do is download, customize and print and use in your Human Resource Department or Payroll Department.
If an employee from your organization has their payroll deposited directly into their personal bank account instead of receiving a paper payroll check and then your organization has to stop this direct deposit for any type of reason then you might need one of the forms below.
The Revocation of Prior Authorization for an Automatic (Direct) Deposit form below is what you provide to any third party initiating any automatic direct deposits to your old account and includes:
- Company Name and Address
- Statement - I/we revoke all prior authorizations of the company (defined above) to initiate credit entries to any of my/our accounts at any financial institution. I/we authorize the Company to initiate credit entries, and to initiate any debit entries needed to correct erroneous credit entries, to my/our account (identified below) at the Financial Institution (identified below) for the purpose of automatically depositing funds in the account. I/we acknowledge that the origination of these transactions must comply with the U.S. law.
- Account - Checking/Share Draft, Savings/Share Savings, Other
- Account Number
- Taxpayer ID Number(s)
- Signature(s) - Print Name and Date
- Note - *Provide this form to any third party initiating any automatic direct deposits to your old account.
Direct Deposit Benefits
There are many benefits to the employee if he or she decides to do direct deposit such as:
- No more rushing to the bank before they close to deposit payroll check.
- Knowing your paycheck will be already in the bank every pay period on the same payroll date and sometimes earlier depending on if payroll date falls on holidays or weekends.
- No more worrying about check getting lost in the mail.
- No more worrying about losing your check.
- If you are out of town, you know your money will be there for you to use your ATM or bank debit card. Your funds will always be in your account when you need them.
