Guardian Consent Form and More

The Guardian Consent is a very important printable form that you have to fill out and get notarized by a Notary Public in order for someone such as the Grandmother, Grandfather, etc. to be able to have temporary legal guardianship over the child in order to bring he or she to doctor, hospital, church camp or anywhere else that needs parents/guardians signature.
The Photo Consent Form is what you would need to use if you were planning on using a child's photo on a form, the internet like on a website or anywhere else in the public's view. This form would have to be filled out by the parents or guardians along with the parents or guardians signature.
The Church Camp Consent Form is another very important form for your church to have available if your church has a youth camp or any kind of camp activities.
The Church Camp Consent form includes:
- Child's Name, Date of Birth and Age
- "I, parent/guardian* of the above named child (“Child”), consent to my child participating in the church camp organized by the _______________________________ Church from _____________ 20__ to _____________ 20__ (“Church Camp”)."
- "I agree that my Child’s participation in the Church Camp is purely voluntary. I agree not to hold the Church liable or responsible for any loss or injury sustained by my Child arising in connection with his/her* participation in the Church Camp."
- "I can be contacted at ________________________ (telephone number) in the event of an emergency."
- "(Applicable where Child is under 12 years old)
Additionally, where my child is under 12 years old (i.e. DOB on and after ____________), I have nominated _____________________________ (name of nominee), and he or she* has agreed to be the guardian of my Child in my absence during the Church Camp." - Signature of Parent or Guardian
- Date of Signature
