The Human Resources Request sample forms below are great necessary tools and an asset for your church or organization's Human Resources Department to use in their office.
The PTO (Paid Time Off) Request sample form below is used by employees to request paid time off in accordance to the church or organization's policy and procedures because certain types of paid time off might require prior notification and approval of supervisory staff and/or the Human Resources department.
The PTO (Paid Time Off) Request form is used for numerous reasons such as if the employee is sick and/or had a deceased loved one and had to take off and needs to use their PTO time to cover the hours and/or days that they missed from work. In other circumstances, the employee might have received a subpoena to have to go to court to do jury duty on days and even at times they have to go weeks at a time or they are just simply wanting to take time off for their birthday or other personal reasons.
If you have employees and/or volunteers in your church and/or organization then you will probably need this form to help organize and keep your office functioning properly. The PTO Request form includes:
- Company Name
- Employee Name
- Date of Request
- Department
- Date(s) of Requested Paid Time Off
- Comments
- Employee's Signature and Date of Signature
- Supervisor's Signature and Date of Signature
The Vacation Request form is used by employees when they are seeking to submit vacation time and must complete and submit this form prior to taking their vacation. Depending on your organization's policy and procedures, they might have to submit this request as much as months prior to their desired vacation dates of time off. The Vacation Request form includes:
- Employee Name
- Date of Request
- Vacation Date(s)
- Alternate Choice for Vacation Date(s)
- Extra Vacation Date(s) if it Occurs During Holiday
- If You Prefer to Split Vacation Date(s)
- Fill Out and Return to
- Comments
- Office Use: Approved Signature, Date(s)
