Does your church have fundraisers to raise money for special causes?
If yes, then you might want these fundraiser forms below to help your church fundraising coordinator to get the proper approvals from administration before and after the fundraising event.
The key to a successful fundraising event is to be very well prepared and organized before, during and after the event.
The accounting form (#1) below for church fundraising includes:
- Name of Church
- Name of Form - Fundraising Accounting Form
- Name and Date of Event
- Name of Organizing Group or Ministry
- Estimated # of participants/Attendees
- Event Leader and Contact Information
- Number of Event Volunteers
- Estimated # of Volunteer Hours
- Income Details - e.g., ticket sales, food, silent auction, etc. and total income
- Estimated Value of any Non-Cash Donations - Please detail number of items and quantity - Please do not add this estimate to total income.
- Expense Details - e.g., food, equipment rental, supplies, postage, etc. and total expenses
- Net Event Proceeds- Income minus Expenses
- Name of Person Who Completed the Form
- Current Date
The accounting form (#2) below for church fundraisers includes:
- Name of Church
- Name of Form - Fundraiser Accounting Form
- Statement - Please complete and distribute this form BEFORE your event (with estimates only) and AFTER your event (same form as estimates, but include actuals) to:
- Name and Date of Event
- Name of Account or Club
- Account Number
- Event Leader and Contact Information
- Fundraiser Monies will be used for
- Estimate Before and Actual After - A. Number
of items to be purchased/expected attendees, B. Cost per items/person, C. Total Cost (A X B), D. Number
of items to be sold/expected attendees, E.
Cost per items/person, F. Total Revenue (D X E), G. Revenue (Line F), H. Cost (Line C), I. Profit - Explanation of difference between estimate and actual profit
- Approval of Fundraiser (BEFORE the event):- Event Organizer's Signature and Date, Department Chair or Club Advisor's Signature and Date, Division Director of Fundraiser Activities Signature and Date
- Approval of Actuals (AFTER the event) - Department Chair or Club Advisor's Signature and Date and Division Director of Fundraiser Activities Signature and Date
- Note - ALL deposits for this event should contain the same description as above.
